V\u011bdc\u016fm z \u00dastavu \u017eivo\u010di\u0161n\u00e9 fyziologie a genetiky AV \u010cR se poda\u0159ilo z krve izolovat mal\u00e9 \u010d\u00e1stice, kter\u00e9 jsou schopn\u00e9 n\u00e9st informaci o neurodegenerativn\u00edm onemocn\u011bn\u00ed \u2013 Huntingtonov\u011b chorob\u011b. Z\u00e1rove\u0148 je obt\u00ed\u017en\u00e9 p\u0159\u00edmo sledovat slo\u017een\u00ed mozkov\u00e9 tk\u00e1n\u011b b\u011bhem \u017eivota jedince, proto\u017ee je to org\u00e1n v podstat\u011b nedostupn\u00fd pro biopsie.<\/p>\n<p>V\u011bdc\u016fm z centra Pigmod \u00dastavu \u017eivo\u010di\u0161n\u00e9 fyziologie a genetiky AV \u010cR v Lib\u011bchov\u011b, kte\u0159\u00ed se dlouhodob\u011b zab\u00fdvaj\u00ed v\u00fdzkumem z\u00e1va\u017en\u00fdch onemocn\u011bn\u00ed mozku, se poda\u0159ilo izolovat a popsat mal\u00e9 v\u00e1\u010dky, kter\u00e9 jsou snadno z\u00edskateln\u00e9 z krve a mohou n\u00e9st informaci o posti\u017een\u00ed mozkov\u00e9 tk\u00e1n\u011b.<\/p>\n<h6>Mal\u00e9 v\u00e1\u010dky z krve mohou n\u00e9st informaci o stavu vzd\u00e1len\u00fdch org\u00e1n\u016f<\/h6>\n<p><em>\u201eP\u0159i\u0161li jsme na to, \u017ee v mal\u00fdch v\u00e1\u010dc\u00edch z krve, tzv. extracelul\u00e1rn\u00edch v\u00e1\u010dc\u00edch \u010di exosomech, je p\u0159\u00edtomen huntingtin, tedy b\u00edlkovina, jej\u00ed\u017e mutace je p\u0159\u00ed\u010dinou Huntingtonovy nemoci,\u201c<\/em> popisuje v\u00fdsledky pr\u00e1ce vedouc\u00ed t\u00fdmu Helena Kupcov\u00e1 Skaln\u00edkov\u00e1 z \u00dastavu \u017eivo\u010di\u0161n\u00e9 fyziologie a genetiky AV \u010cR. <em>\u201eCelkov\u00e9 mno\u017estv\u00ed huntingtinu ve v\u00e1\u010dc\u00edch je vy\u0161\u0161\u00ed u prase\u010d\u00edch model\u016f pacient\u016f s Huntingtonovou nemoc\u00ed v porovn\u00e1n\u00ed se zdrav\u00fdmi kontroln\u00edmi jedinci. Nav\u00edc byly rozpozn\u00e1ny specifick\u00e9 formy mutovan\u00e9ho huntingtinu, kter\u00e9 se u zdrav\u00fdch jedinc\u016f nenach\u00e1zely,\u201c<\/em> dod\u00e1v\u00e1 v\u011bdkyn\u011b.<\/p>\n<p>V posledn\u00edch letech se extracelul\u00e1rn\u00ed v\u00e1\u010dky staly objektem velk\u00e9ho z\u00e1jmu v\u00fdzkumn\u00edk\u016f \u2013 pr\u00e1v\u011b proto, \u017ee se daj\u00ed pom\u011brn\u011b snadno z\u00edskat z krve \u010di jin\u00fdch t\u011bln\u00edch tekutin, a obsahuj\u00ed r\u016fzn\u00e9 molekuly v\u010detn\u011b RNA \u010di b\u00edlkovin, kter\u00e9 by mohly b\u00fdt vyu\u017eity k diagnostice r\u016fzn\u00fdch onemocn\u011bn\u00ed.<\/p>\n<p><em>\u201eM\u016f\u017eeme si to p\u0159edstavit, jako by se malinkat\u00e9 kous\u00ed\u010dky nemocn\u00e9 tk\u00e1n\u011b ze vzd\u00e1len\u00fdch a t\u011b\u017eko dostupn\u00fdch org\u00e1n\u016f dostaly do krve a z odb\u011bru krve, bez nutnosti biopsi\u00ed dan\u00fdch org\u00e1n\u016f, by \u0161lo vy\u0161et\u0159it tuto nemocnou tk\u00e1\u0148 a zjistit \u010di sledovat pr\u016fb\u011bh a \u00fa\u010dinnost l\u00e9\u010dby nap\u0159. neurodegenerativn\u00edch nemoc\u00ed \u010di jin\u00fdch onemocn\u011bn\u00ed, v\u010detn\u011b n\u00e1dorov\u00fdch,\u201c<\/em> vysv\u011btluje potenci\u00e1ln\u00ed v\u00fdznam v\u00e1\u010dk\u016f Helena Kupcov\u00e1 Skaln\u00edkov\u00e1.<\/p>\n<h6>Dlouh\u00e1 cesta k opravdov\u00fdm biomarker\u016fm<\/h6>\n<p>Pr\u00e1ce v\u011bdc\u016f centra Pigmod je v\u0161ak teprve na \u00fapln\u00e9m za\u010d\u00e1tku. K vyu\u017eit\u00ed extracelul\u00e1rn\u00edch v\u00e1\u010dk\u016f jako zdroje biomarker\u016f neurodegenerativn\u00edch onemocn\u011bn\u00ed vede je\u0161t\u011b dlouh\u00e1 cesta. Jedn\u00edm z p\u0159edpoklad\u016f je \u00fasp\u011b\u0161n\u00fd z\u00e1kladn\u00ed v\u00fdzkum molekul\u00e1rn\u00edho slo\u017een\u00ed t\u011bchto \u010d\u00e1stic. D\u00edky tomu, \u017ee se v\u011bdc\u016fm poda\u0159ilo do laborato\u0159e zav\u00e9st metody pro izolaci a charakterizaci v\u00e1\u010dk\u016f z krve, mohou je d\u00e1le vyu\u017e\u00edt k hled\u00e1n\u00ed dal\u0161\u00edch molekul (vedle samotn\u00e9ho huntingtinu), kter\u00e9 by mohly odr\u00e1\u017eet v\u00fdvoj nemoci a \u00fa\u010dinnost l\u00e9\u010dby. P\u0159i pr\u00e1ci s extracelul\u00e1rn\u00edmi v\u00e1\u010dky se v\u0161ak v\u011bdci museli vypo\u0159\u00e1dat s t\u00edm, \u017ee v\u00e1\u010dk\u016f se d\u00e1 z krve z\u00edskat jen velmi mal\u00e9 mno\u017estv\u00ed a obt\u00ed\u017en\u011b se s nimi pracuje, proto\u017ee nejsou vid\u011bt a b\u011b\u017en\u00e9 techniky je neodhal\u00ed.<\/p>\n<p><em>\u201eMo\u017en\u00e1 to zn\u00ed jednodu\u0161e, ale technicky je pr\u00e1ce s v\u00e1\u010dky velmi slo\u017eit\u00e1. P\u0159edstavte si, \u017ee deset hodin s n\u011b\u010d\u00edm pracujete a na z\u00e1v\u011br vlastn\u011b v\u016fbec nev\u00edte, zda ve vzorc\u00edch v\u00e1mi sledovan\u00e9 \u010d\u00e1stice opravdu m\u00e1te. V\u00e1\u010dky jsou toti\u017e tak mal\u00e9, \u017ee nejsou vid\u011bt ani ve sv\u011bteln\u00e9m mikroskopu a je jich tak m\u00e1lo, \u017ee zkumavka s nimi se jev\u00ed jako zcela pr\u00e1zdn\u00e1,\u201c<\/em> popisuje \u00faskal\u00ed pr\u00e1ce s extracelul\u00e1rn\u00edmi v\u00e1\u010dky Helena Kupcov\u00e1 Skaln\u00edkov\u00e1. <em>\u201eD\u00edky spolupr\u00e1ci s \u00dastavem experiment\u00e1ln\u00ed medic\u00edny AV \u010cR a s Vysokou \u0161kolou chemicko-technologickou jsme v\u0161ak m\u011bli p\u0159\u00edstup ke speci\u00e1ln\u00edm technik\u00e1m pro detekci nano\u010d\u00e1stic, kter\u00e9 n\u00e1m umo\u017enily ur\u010dit po\u010det a velikost vyizolovan\u00fdch v\u00e1\u010dk\u016f,\u201c<\/em> dopl\u0148uje v\u011bdkyn\u011b.<\/p>\n<p>V\u011bdci nyn\u00ed pracuj\u00ed na anal\u00fdz\u00e1ch molekul\u00e1rn\u00edho slo\u017een\u00ed v\u00e1\u010dk\u016f a porovn\u00e1vaj\u00ed obsah v\u00e1\u010dk\u016f mezi miniprasaty nesouc\u00edmi gen pro mutovan\u00fd huntingtin jako p\u0159\u00ed\u010dinu Huntingtonovy nemoci, a kontroln\u00edmi jedinci. Z\u00e1rove\u0148 ve spolupr\u00e1ci s Neurologickou klinikou a Centrem klinick\u00fdch neurov\u011bd 1. l\u00e9ka\u0159sk\u00e9 fakulty Univerzity Karlovy a V\u0161eobecn\u00e9 fakultn\u00ed nemocnice analyzuj\u00ed slo\u017een\u00ed v\u00e1\u010dk\u016f izolovan\u00fdch z krevn\u00ed plazmy pacient\u016f s Huntingtonovou nemoc\u00ed a kontroln\u00edch osob.<\/p>\n<p><em>\u201eNa\u0161e v\u00fdsledky prok\u00e1zaly, \u017ee extracelul\u00e1rn\u00ed v\u00e1\u010dky nesou informaci o Huntingtonov\u011b nemoci. Pokud se poda\u0159\u00ed nal\u00e9zt dal\u0161\u00ed molekuly, kter\u00e9 specificky odr\u00e1\u017eej\u00ed po\u0161kozen\u00ed tk\u00e1n\u00ed, mohlo by to pomoci urychlit v\u00fdvoj nov\u00fdch terapi\u00ed a usnadnit sledov\u00e1n\u00ed jejich \u00fa\u010dinnosti,\u201c<\/em> vyzdvihuje potenci\u00e1l extracelul\u00e1rn\u00edch v\u00e1\u010dk\u016f Helena Kupcov\u00e1 Skaln\u00edkov\u00e1.<\/p>\n<hr \/>\n<p>&nbsp;<\/p>\n<ul>\n<li>Huntingtonova choroba je vz\u00e1cn\u00e9 d\u011bdi\u010dn\u00e9 dosud nel\u00e9\u010diteln\u00e9 neurodegenerativn\u00ed onemocn\u011bn\u00ed mozku, kter\u00e9 postihuje 8 ze 100 000 lid\u00ed. Projevuje se charakteristick\u00fdmi trhav\u00fdmi pohyby kon\u010detin a cel\u00e9ho t\u011bla, kter\u00e9 postupn\u011b p\u0159ech\u00e1zej\u00ed a\u017e v \u00fapln\u00e9 ochrnut\u00ed svalstva. Poruchy hybnosti b\u00fdvaj\u00ed doprov\u00e1zeny poruchami chov\u00e1n\u00ed a sn\u00ed\u017een\u00edm intelektu\u00e1ln\u00edch schopnost\u00ed. Zr\u00e1dnost onemocn\u011bn\u00ed spo\u010d\u00edv\u00e1 v jeho postupn\u00e9m a nezvratn\u00e9m rozvoji, zpravidla a\u017e v obdob\u00ed dosp\u011blosti, okolo 40 roku \u017eivota.
